After putting on the wrist strap do I just attach it to bare metal INSIDE the case or OUTSIDE or do I just put it into an grounded outlet? also should I leave the PSU plugged in? when I use a wrist strap can I use both hands or only the one I put it on?

You can attach the grounding strap to any bare metal inside or outside the case (the case is grounded to the motherboard and power supply).
You're best to unplug the computer. By using the wriststrap, you have the same electric potential as the case/motherboard, so there will be no static discharge when you insert or remove an item.
Power supplies have potentially high voltage, but no significant amount of static electricity.
A wriststrap typically includes a 1 megohm or higher resistor between the wrist and the clamp. that way, if you happen to connect the clamp to a live circuit, you don't light up like a simple copper wire.

1.) it doesn't matter if inside or outside; it simply needs to be a part that is Grounded (basically but not limited to meaning "on the ground"), so that the static discharge can surge from you towards it and through the strap, once you gain the static from something else.
2.) No, you must not leave the PSU plugged in. it is the greatest source of static electricity in your vicinity.
3.) In reality, this is a useless precautionary measure. I've built dozens of systems without a strap and never had a component die on me as a result of static discharge.
The best procedure is to connect the chassis' of the two computers together and connect to a grounded outlet with the power switched off. Connect the wrist strap to the grounded chassis of one of the computers.
